What could be cause for spotting?

I have copper t inserted after my fist kid. It's got expired in March but i was not able to remove it and I had spotting on 20th nov on my period day which is stopped in 1-2 days. I visited doctor on 27th and gone for ultrasound and pregnancy test but I was not pregnant and ultrasound was normal. So doctor removed copper t on 1dec. I again had bleeding on 5th dec which stopped next day. What could be the reason for bleeding?
